Physicians & Clinicians
Collaborating to Deliver Whole‑Child Palliative & Hospice Care
Community PēdsCare® is North Florida’s only comprehensive pediatric palliative, hospice, and supportive‑care program. We partner with referring physicians, advanced practitioners, hospitalists, and specialty teams to ensure every child with a life‑limiting or life‑threatening condition—and every family member—receives seamless, evidence‑based care in the setting that best meets their needs. From prenatal consultations to young adulthood, our interdisciplinary team provides expert symptom management, psychosocial support, and resources that honor each family’s goals, culture, and values.
Why Community PēdsCare®?
The Community PēdsCare® program provides children and their families with a continuum of care that extends beyond the physician’s office and goes with the child through their care journey.
Referring a child to pediatric hospice care benefits not just the patient and family, but also the physicians and clinical teams involved in their care. Here’s how:
Clinical Benefits to Referring Physicians/Clinicians
Enhanced Quality of Care
- Hospice teams specialize in managing complex symptoms (pain, respiratory distress, nausea, etc.), improving patient comfort.
- Families receive 24/7 support, reducing urgent calls to the referring team and preventing unnecessary ER visits or hospitalizations.
Collaborative Support Model
- Hospice becomes an extension of the care team, often involving a multidisciplinary approach (nurses, social workers, chaplains, etc.).
- Physicians can maintain involvement and decision-making without having to handle day-to-day logistics.
Continuity of Care
- Helps ensure that the child’s care plan is honored across all settings — home, hospital, hospice facility — without fragmentation.
Emotional & Ethical Benefits
Reduces Moral Distress
- Clinicians may struggle when curative treatment is no longer appropriate. Hospice supports care that aligns with goals and values, helping resolve ethical dilemmas.
Supports Goals-of-Care Conversations
- Hospice teams are trained in guiding difficult conversations with families, helping clarify care goals and reduce conflicts or confusion.
Time & Workflow Efficiency
Decreases Burden on Office/Clinic Resources
Hospice assumes primary responsibility for home-based care coordination, prescriptions, equipment, etc., freeing up the primary team.
Streamlines Complex Care Cases
Physicians can focus on medical oversight and consultations rather than daily care management, especially when patients have escalating needs.
Professional Development & Reputation
Improves Patient & Family Satisfaction
High-quality end-of-life support reflects positively on the entire care team, fostering trust and appreciation from families.
Demonstrates Adherence to Best Practices
Professional societies (like AAP, CAPC) recommend early palliative and hospice integration. Referring aligns with evidence-based standards.
We’re here to support you every step of the way. Together, we can ensure your family receives care and support.
