About Us
When a child lives with a chronic, complex, or life-limiting illness, the entire family faces difficult care decisions and challenges that can feel overwhelming. At Community PedsCare®, we are here to lighten that burden by providing a continuum of care that extends far beyond the physician’s office or hospital walls. Our nationally recognized pediatric program offers holistic support for children and teens with serious illnesses while embracing their families every step of the way.
From the prenatal stage through age 21, we strive to deliver compassionate, family-centered care that addresses the unique needs of each child. Our expert team focuses on providing the best treatment for pain and distressing symptoms, helping children find comfort and relief. At the same time, we offer essential emotional and practical support to families coping with the stress and uncertainty of their child’s illness.
Through a holistic approach, Community PedsCare® adds a crucial layer of support that enhances the quality of life for both the child and their loved ones. Our mission is to be more than caregivers—we aim to be partners in your journey, helping your family navigate challenges with compassion, expertise and hope.
Our History
The history of pediatric palliative care in Jacksonville, Florida, began with the establishment of the Community PedsCare program in 2001 by Community Hospice of Northeast Florida.
This program was created to provide comprehensive, community-based palliative and end-of-life care to children with complex, life-threatening conditions and their families. It was developed in collaboration with Wolfson Children’s Hospital, Nemours Children’s Clinic, and the University of Florida, and has since been recognized for its quality and has expanded its services to North Central Florida.
- 2001: Community PedsCare is established by Community Hospice of Northeast Florida to provide palliative and end-of-life care to children in the community.
- Early Development: The program was created in collaboration with Wolfson Children’s Hospital, Nemours Children’s Clinic, and the University of Florida to address the specific needs of children with life-limiting illnesses.
- Services: Community PedsCare offers a range of services including pain and symptom management, psychosocial and spiritual support, and family respite care. It also provides bereavement and grief support, with special attention to siblings.
- Recognition: The program was a recipient of the 2008 Quality in Palliative Care Leadership Award for implementing palliative care principles.
- Expansion: The program has expanded its reach to serve children throughout North Central Florida and has received national recognition.
- Ongoing Support: Community PedsCare continues to provide care to children up to age 21, focusing on a family-centered approach regardless of a family’s financial status or insurance coverage.
